COPDeducation have been working in partnership with Novartis to produce 2 documents that will help you deliver better care for your patients with COPD. Novartis representatives in primary and secondary care will distribute the materials or they can be downloaded from our website. Both documents have an insert (COPD review template or self management plan) and extra copies will only be available via download from these pages

Introduction

An important part in the delivery of quality care for your patients includes time spent in delivering disease specific education and help in self-management. These 2 documents work synergistically to deliver these objects which are an important part of the national strategy and NICE quality standards.

The booklet

The self-management plan booklet contains education material for patients diagnosed with COPD. Topics include : What is COPD, diagnosis, treatments and exacerbations.

It is used with the self -management plan inserted into the back of the booklet.

Detailed instruction for patients and clinicians is included.

Self Management Plan

The self-management plan template that accompanies the booklet should be completed with the patient. This plan should be used in conjunction with the annual review template to help deliver the NICE quality standards 2011.
